Forming or Changing Dissertation Committee

    Complete this form and submit it to the JDP Program Manager to obtain JDP approval for your proposed committee before you submit the OGE form for committee approval. This JDP form is normally completed at least 3 weeks before defense of the dissertation proposal.  This form can also be done via DocuSign which eliminates the need to track down signatures from your committee members.     If you need to change your dissertation committee, complete this form and have all former and proposed members sign it. Return to the JDP Office for approval.
