Non-Coursework Requirements

Minimum Requirements
All Ph.D. candidates must complete the following requirements:
(c) doctoral residency requirements
(e) a supervised minimum 1500 (NASP) or 2000 (APA) hour advanced full-time Internship (required for Licensure students and optional for Pathways students).
- Students must complete all other coursework, pass doctoral comprehensives, and successfully propose their dissertation study prior to starting Internship.
The recommended course sequence for Ph.D. students varies depending on prior coursework, identified interest, and the selected cognate area.

Forms

General:
Transfer of Credit - Completed when bringing in credits from another University
Annual Review Form - Completed by faculty
Background Check Form - Completed upon acceptance into the program
Residency Documentation Form - Typically completed your 1st or 2nd year in the program
Course Substitution or Waiver Form - Complete when you have taken a course that fills the place of one on your coursework plan
Continuous Enrollment - After all courses are complete and working on dissertation
Independent Study - Dissertation credits or other (discuss with your advisor)

- Announcement of Oral Proposal/Defense Form - Submit to ASA
- Proposal Approval Form - Faculty completes, submit to ASA for recordkeeping
- Committee Recommendation Form - Signed by committee members, adhere to university deadlines, submit to OGE
- Schedule of Oral Defense - Due 4 weeks prior to defense, adhere to university deadlines, submit to OGE
- Manuscript Format & Style Form - Formatting guidelines and instructions for submitting to the university
- Checklist of Dissertation Completion of Tasks & Forms - For student use, guide for general order of task completion
Licensure:
Fully complete forms and submit to ASA for departmental signatures.
